completed Registration Forms with approved payment:
a confirmation of registration will be sent to the billing contact
along with one
'Participant Workshop Selection Form'
and online access code for each paid registrant.
Purchase Orders will be accepted for Regular Registra-
tions and On-site Registrations only. Online registrants can print a
quote with their purchase
order number.

1. To register individuals
and groups send completed

Groups of 5 or more participants on one
cheque will receive a 10% dscount on overall registrations.
All cancellations must submit a cancellation request form no later than
January 25, 2012.
A cancellation fee of $100 will be billed for each registration for
Administration fees.
No refunds,
Substitutions are acceptable, however, all conference info/handouts will be in
the original participants name.
Registrants who do not show up at the conference and who have not submitted a cancellation form by January 25, 2012 will be still held responsible for paying the total conference fee because all conference costs are already paid for using each participants registration costs.
